Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to patient care, excellent clinical skills, and the ability to work effectively within diverse healthcare settings. As an LPN, you will play a vital role in providing high-quality nursing care, supporting patient recovery, and ensuring compliance with healthcare standards. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories, utilizing physiology knowledge and medical terminology.
- Administer medications and injections accurately while adhering to HIPAA regulations and sterile technique protocols.
- Assist with medical procedures such as catheterization, tube feeding, venipuncture, and wound suturing under supervision.
- Monitor patient responses to treatments such as IV infusion, ventilator management, and pain management strategies.
- Document all patient interactions thoroughly within Electronic Health Record (EHR) systems like Epic, Cerner, or Athenahealth to ensure accurate medical records.
- Collaborate with multidisciplinary teams on discharge planning, case management, and post-acute care coordination for optimal patient outcomes.
- Provide specialized care for patients with dementia or Alzheimer's disease in memory care units or assisted living facilities.
- Support infection control practices and adhere to standards for infectious disease care across all settings.
Requirements
- Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
- Proven experience in diverse healthcare environments such as hospitals (including ICU and Level I Trauma Center), nursing homes, hospice care, or outpatient clinics.
- Demonstrated proficiency in medical documentation, ICD-9/ICD-10 coding, and medical terminology.
- Experience with EMR/EHR systems such as Epic, Cerner, or eClinicalWorks is highly desirable.
- Knowledge of critical care procedures including airway management, acute pain management, and trauma response is preferred.
- Ability to perform phlebotomy, venipuncture, catheter care, and injections competently.
- Strong interpersonal skills working with people of all ages—including pediatric (PICU), neonatal (neonatal ventilator), and adult populations—especially those with disabilities or complex health needs.
- Familiarity with home & community care practices such as home health visits or travel nursing is advantageous.
