Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong foundation in patient care, with expertise in various clinical settings, including long-term care, acute care, and community health. As an LPN, you will provide essential nursing services, support patient assessments, administer medications and treatments, and ensure the delivery of high-quality care in accordance with healthcare standards and regulations. Your role is vital in promoting patient well-being and fostering a safe, respectful environment for diverse populations.
Duties
- Perform comprehensive patient assessments by monitoring vital signs, conducting physical examinations, and documenting medical histories accurately using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications, injections, IV infusions, and treatments following prescribed protocols while adhering to HIPAA regulations to maintain patient confidentiality.
- Assist with medical procedures including catheterization, suturing, tube feeding, and specimen collection/processing to support diagnostic evaluation and treatment plans.
- Provide specialized care for patients with complex needs such as dementia, Alzheimer's disease, memory care residents, or those requiring hospice or palliative care.
- Support emergency and acute care situations by managing airway control, ventilator use, and responding effectively during triage or Level I trauma center scenarios.
- Collaborate with multidisciplinary teams in settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics to coordinate discharge planning and case management efforts.
- Educate patients and families on health coaching topics like medication administration, nutrition, injury prevention, and disease management tailored to individual needs.
- Maintain accurate medical documentation of patient care activities while ensuring compliance with all regulatory standards including ICD-9/ICD-10 coding for diagnoses.
Skills
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks for documentation and patient records management.
- Critical care experience including ICU or Level II trauma center background is highly desirable.
- Strong knowledge of physiology, anatomy, pharmacology, and medical terminology to support comprehensive patient assessments.
- Experience administering injections, phlebotomy (venipuncture), catheter care, and sterile processing techniques.
- Familiarity with diverse clinical environments such as emergency medicine, urgent care, hospital medicine, or outpatient clinics.
- Ability to work effectively with individuals across the lifespan including pediatrics (neonatal ventilator use), adults in internal medicine or geriatrics settings (including dementia and Alzheimer’s care).
- Excellent communication skills for triage situations and working with people with disabilities or developmental challenges.
- Knowledge of infection control practices along with aseptic techniques to ensure patient safety during invasive procedures.
