Job Description

The Medical Social Worker plays a vital role in providing comprehensive support to patients and their families within a healthcare setting. This position involves assessing the social, emotional, and financial needs of patients, particularly in assisted living and outpatient environments. The Medical Social Worker will collaborate with healthcare teams to develop care plans that address the psychosocial aspects of patient care, ensuring that individuals receive the necessary resources and support for optimal health outcomes. Duties

  • Conduct assessments to identify patients' social, emotional, and environmental needs.
  • Provide individual and group counseling to patients dealing with various challenges, including addiction and mental health issues.
  • Develop and implement treatment plans that incorporate motivational interviewing and behavioral therapy techniques.
  • Facilitate communication between patients, families, and healthcare providers to ensure coordinated care.
  • Advocate for patients' rights and access to community resources, including financial assistance programs.
  • Maintain confidentiality and adhere to HIPAA regulations while documenting patient interactions and treatment plans.
  • Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans as needed.
  • Offer support in pediatrics when necessary, addressing the unique needs of younger patients.
  • Prepare and attend bimonthly IDG meeting.
Skills
  • Proficiency in individual group counseling techniques.
  • Experience working in assisted living facilities or similar environments.
  • Knowledge of pediatric care practices is a plus.
  • Familiarity with addiction counseling methodologies.
  • Strong background in psychotherapy practices.
  • Understanding of HIPAA regulations to ensure patient confidentiality.
  • Ability to utilize motivational interviewing strategies effectively.
  • Experience in outpatient settings providing behavioral therapy services.
  • Excellent communication skills for effective collaboration with healthcare teams and patient advocacy.
