Job Description:

• Conduct psychosocial assessments of patients and their families in hospice or medical care settings
• Provide counseling, emotional support, and crisis intervention for patients and families
• Assist patients and families in understanding diagnoses, treatment options, and end-of-life care decisions
• Coordinate with physicians, nurses, and the interdisciplinary care team to develop and implement care plans
• Facilitate access to community resources, financial support, and social services
• Advocate for patients' rights and ensure patient-centered care
• Maintain accurate documentation, case notes, and reports in compliance with organizational and legal standards
• Conduct patient and family education sessions on coping strategies, bereavement, and caregiving
• Collaborate with external agencies, volunteers, and support groups as needed
